Samenvatting

Dit afstudeerproject betreft het produceren van een voorlichtingsvideoband voor patiënten die een arthroscopie van de knie ondergaan. De opdrachtgever is het St. Annaziekenhuis te Geldrop. Er bestaat reeds een voorlichtingsvideoband, echter deze bevat onduidelijkheden, mist belangrijke informatie en moet om deze reden worden aangepast. De tweede opdracht betreft een literatuuronderzoek naar de meest effectieve manier van voorlichting aan patiëntengroepen. De opdrachtgever hiervan is de Fontys Paramedische Hogeschool Eindhoven. De opdrachten zijn vertaald naar de volgende hoofdvraag:

Hoe kan, op basis van beschikbare wetenschappelijke literatuur, een productie van een voorlichtingsvideoband tot stand komen voor patiënten die een arthroscopie van de knie moeten ondergaan?

De gehanteerde methode die leidt tot het beantwoorden van de hoofdvraag, betreft een tweedeling. In verband met het tijdstip van opname dat zeer vroeg in het onderzoekstraject gepland was, is begonnen met het onderbouwen van de voorlichtingsvideoband met behulp van medische literatuur. Achteraf vond de wetenschappelijke onderbouwing van voorlichtingskundige aspecten plaats.

De medische inhoud van de reeds bestaande voorlichtingsvideoband is beoordeeld aan de hand van: de brochure "arthroscopie", de resultaten van het patiëntentevredenheidsonderzoek, het interview met Ellis Bos (fysiotherapeute in het St. Annaziekenhuis) en het zoeken naar zowel wetenschappelijke medische literatuur als literatuur in gevallen waarbij de praktijk gebaseerd is op ervaringen en kennis van fysiotherapeuten, artsen en verplegend personeel, verbonden aan het St. Annaziekenhuis. De resultaten van deze beoordeling zijn verwerkt in de inhoud van de nieuwe voorlichtingsvideoband.

Na de filmopnames van de voorlichtingsvideoband " Voorlichtingsvideoband arthroscopie van de knie" is het literatuuronderzoek voortgezet met het zoeken naar wetenschappelijke informatie over de meest effectieve manier van voorlichting aan patiëntengroepen. Er is gezocht naar informatie over patiëntenvoorlichting in algemene zin, patiëntenvoorlichting en emoties en patiëntenvoorlichting in specifieke zin.

Het afstudeerproject is sterk beïnvloed door het vroege tijdstip van de opnamedag in het onderzoekstraject. De projectgroep is van mening dat de voorlichtingsvideoband van grotere kwaliteit had kunnen zijn, wanneer eerst was gestart met het literatuuronderzoek naar de meest effectieve manier van voorlichting aan patiëntengroepen. De voorlichtingsvideoband houdt geen rekening met het feit dat bepaalde voorlichting, de emoties en het gedrag van patiënten kan beïnvloeden. Daarnaast is gebleken dat hoe meer zintuigen worden geprikkeld, des te beter de informatie wordt opgenomen. Hiermee is geen rekening gehouden met de eerste versie van de voorlichtingsvideoband.

Het productverslag geeft een antwoord op de vraag wat de meest effectieve manier van voorlichting aan patiëntengroepen is. De voorlichtingsvideoband voldoet aan de definitie van patiëntenvoorlichting; deze speelt in zijn geheel in op kennis/ inzicht/ vaardigheden/ attitude en gedrag niveau. De voorlichtingsmethoden die beschreven worden zijn schriftelijk en audiovisueel materiaal. De projectgroep is van mening dat het gebruik van internet nog volop in ontwikkeling is. Op korte termijn wordt het gebruik van een voorlichtingsvideoband aanbevolen. Voorwaarde daarbij is dat de organisatie goed geregeld moet zijn zodat de gehele doelgroep bereikt wordt. Als overgang naar het internet op langere termijn, kan gebruik worden gemaakt van een cd-rom als voorlichtingsmedium.

Vervolgens geeft de projectgroep een aanbeveling om het effect van de voorlichtingsvideoband te meten. Deze aanbeveling betreft het uitvoeren van een effectiviteitsonderzoek naar de 'Voorlichtingsvideoband arthroscopie van de knie'. De projectgroep is van mening dat de geproduceerde videoband onderworpen moet worden aan het reeds bestaande patiententevredenheidsonderzoek. Op deze manier wordt een betrouwbaar resultaat bereikt.


This end term paper concerns the production of a video tape in order to inform patients who are being prepared for an arthroscopy of the knee. The assignment was provided by St. Anna's Hospital (St. Annaziekenhuis) in Geldrop. There already are video tapes available concerning this specific topic, however, the existing material is vague, lacks of important information and for that reason has to be improved. The project is supported by a literary study that must indicate the most effective way of informing certain patient groups. This problem results in the following research question:

What is the most effective way to produce a video tape that informs patients who are being prepared for an arthroscopy of the knee, based on the available scientific literature?

To be able to formulate an adequate answer to the research question, we used a dual research method. Since the recording day for the video tape was set on a date very early in the course of the research, we started with the literary study about the medical content of the informative video tape. The scientific argumentation of informational aspects took place after the production of the video tape.

The already existing tape was first evaluated by means of the brochure "arthroscopy", the results of the patient's satisfactory poll, the interview with Mrs. Ellis Bos (physiotherapist at St. Anna's Hospital) and by means of the search for both scientific literature and literature based on experiences and know-how of physiotherapists, doctors and nurses at St. Anna's Hospital. The results of this evaluation are incorporated in the medical content of the new informative tape.

After the recording of the tape "Informative video tape arthroscopy of the knee", the literary study continued with the search for information about the most effective way of informing patient groups. The information sought, concerned general patient information, patient information in groups and several information methods.

The study was strongly influenced by the moment that the information tape had to be recorded, since it was scheduled on a very early moment in the course of the research. This leads to the fact that the possibility might exist that the tape would have had more quality if the literary study about the most effective way of giving information to patient groups, was completed before recording the tape. The tape does not consider the fact that certain information influences the emotions and the behaviour of patients. Besides, the results show that the more senses are being stimulated, the more effective the information is perceived. This factor was not incorporated in the first version of the video tape.

The product report answers to the question about the most effective way of informing patient groups. The video tape meets with the requirements of the definition of patient information, that concerns the level of cognition, insight and skills and the level of attitudes and behaviour. The information methods described have a bearing on textual and audio visual material. Since the use of the Internet is still developing, we recommend to use in the short term, for this particular purpose, a video tape that reaches the entire target group. An important condition in this concern is a well arranged and structured organisation or management in order to be able to reach the entire target group. During the transitional phase to the Internet on the long term, it is recommendable to use a cd-rom as information medium.

Finally, in order to measure the efficiency of the renewed and improved video tape, an efficiency poll is recommended. This recommandation concerns the patient satisfactory poll that was used to review the prior informative video tape. By using the same patient satisfactory poll the result will be evaluated as reliable.
